• U.S. Passport must be valid 6 months beyond intended stay • Tickets and Documents for return or onward travel • No Visa required for stay up to three months Requirements for Non-U.S. Citizens to obtain a Tourist Visa for Poland / Visas for stays of more than 90 days • Submit signed valid passport, must have at least 6 months remaining validity beyond intended stay and have at least two blank visa pages • 1 Fully completed and signed Visa Application Form • 2 Passport Photographs • Copy of airline ticket or travel itinerary • Copy of U.S. Alien Registration Card (Green Card) or copy valid U.S. Visa. Poland Business Visa
